Disease Management

Disease management is a set of comprehensive tools that take into account our HealthSource pharmacists expertise not only in the area of pharmaceuticals, but also their full knowledge of disease states. Extensive pharmaceutical expertise combined with full knowledge of disease states selecting the most appropriate therapy can benefit every patient This type of therapy also requires close relationship with patients physician to assure that the best pharmaceutical care is given. Physician may also provide further information to the pharmacy such as lab result data, or procedure report to further enhance patients therapy.

Disease management thus spills into many most commonly treated conditions. Most of these conditions require multiple medications and stringent control and include, but not limited to the following:

04. HIV
05. Pain Management
06. Weight Control

We offer by appointment a comprehensive set of consultation services and tools on the above six disease states.

Fees:

First Session with our pharmacist: $30
Additional sessions as required: $25

Counseling Services:

All patients will receive proper consultation and follow up upon dispensing of medication according to OBRA. If an individual requires further information or additional counseling (in depth consultation that requires more time) they could make an appt. for a future date. Services such as group lectures, brown bag day, and one on one counseling will be made available at additional cost to the patient. Services will focus on specific concerns, teaching tools will be shown, proper use of devices, preparing customized therapy, providing up to date information to both patient and physician. Fee for service schedule will be posted and disclosed to every patient prior to the beginning of sessions.
